This plan is only offered at Smith Family Dental Solutions. It cannot be used at any other dental office.

What your plan will get you

Plan Includes (per patient, per year)

  • 3 Exams total. In addition to your 2 six month check-up exams, you can have one other exam.
    (Example: You have a toothache and you need an emergency exam for the dentist to check it.)

  • 2 Cleanings

  • 1 Panoramic x-ray

  • 1 Set of bite-wing x-rays plus 2 periapical x-rays (PA’s)

  • 1 Diagnostic x-ray (Example: When you come in for your toothache exam, the dentist takes an x-ray of that tooth to help with your diagnosis)

  • 15% Discount on Other Comprehensive Services

Savings Examples

Example 1 (per family member)

You can come in for your 6 month check-up appointment. Services can include:

  • Periodic Exam: $72

  • Cleaning: $123

  • Bitewing x-rays plus 2 PA’s: $164 (This x-ray is good for 1 year.)

  • Panoramic x-ray: $147 (This x-ray is good for 3-5 years.)

The total value is $506 but you pay 0 at time of service because you are on the plan

Example 2 (per family member)

6 months later you come bak for your 2nd check-up.

  • Periodic Exam: $72

  • Cleaning: $123

The total value is $195 but you pay 0 at time of service because you are on the plan

Example 3 (per family member)

You have a toothache. You come in and we do an emergency exam and take an x-ray of your painful tooth.

  • Emergency Exam: $107

  • First x-ray: $42

The total value is $149 but you pay 0 at time of service because you are on the plan

Example 4 (per family member)

It turns out you need a filling on the painful tooth. Since you are on this plan, you will receive a 15% discount on the cost of the filling.
